It is designed for individuals with a range of social, emotional or practical needs who need help navigating their way through the community services and voluntary organisations available locally. The Social Prescribing Link Worker will discuss your concerns through one-to-one appointments, either telephonically, virtually, in a GP surgery or in a community setting. Your can register yourself, or make a referral t=on behlf of someone else, with their consent. When you have completed the form, a member of our team will aim to be in touch in 10 working days

The Social Prescribing Service is primarily a signposting service that will direct and refer individuals to the relevant and more specialist support services. We aim to be the bridge that allows individuals to have knowledge of and access to the variety of services available to them. We do not provide long-term support, and once the referral has been worked on, it will be closed. Patients can re-refer back into the service if they require Social Prescribing support at any point in the future.
